Do you want to cut the foreskin of children? How to nurse at ordinary times?

Outpatient clinics often see parents rushing in with their children, [Doctor, how can a bag bulge up in front of my children when they urinate? [Doctor, do you have a look at my child’s prepuce? Or phimosis? [Doctor, does phimosis need surgery? ]

Excessive foreskin and phimosis are very common in children. Many parents are affected by some online publicity. When they find that their children seem to have excessive foreskin or phimosis, they are extremely worried that it will affect the development of their children’s penis and even feel that it is better to cut off the foreskin immediately.

However, as a urologist, I must be very responsible to tell everyone that children’s redundant foreskin and phimosis should be analyzed in detail and must not be simply and roughly cut off.

Is what prepuce too long and phimosis?

Under normal circumstances, the foreskin covers the penis head, which can protect the penis head and reduce the damage of the outside world to the penis head. At the same time, the foreskin can secrete some sterilizing enzymes and play a certain defensive role.

Redundant prepuceIt means that the penis head is still completely covered by the foreskin when the penis is weak and erect, but the foreskin can be opened by hand to expose the penis head.

PhimosisIt means that the outer mouth of the foreskin is too small and tightly hoop on the penis head, which cannot be turned upward and outward to expose the penis head. Even if you use your hand to open the foreskin, it is difficult to expose the penis head.

In addition to the common redundant foreskin and phimosis, there are two other special circumstances that need parents’ special attention.

One is buried penis, which looks very short in length. Buried penis is mostly caused by obesity, which causes the penis to be blocked by abdominal fat pad.

The other is concealed penis, which is most easily confused with phimosis. Careful examination can find that the foreskin of concealed penis is empty, like a hill, and it is difficult to touch the penis body with hands.

Not all foreskin must be cut!

Do you want to cut the foreskin or not? This requires a specific analysis of the specific situation.

1. The prepuce is too long

First of all, not all redundant foreskin needs surgical treatment. Most children’s redundant foreskin is a physiological phenomenon. At present, the American Academy of Pediatrics (AAP) also recommends that newborns should no longer be circumcised routinely.

Prepuce can play a certain role in covering and protecting the penis head, but too long prepuce is easy to hide dirt and accept dirt, leading to infection and inflammatory adhesion. Chronic inflammatory stimulation in long-term prepuce is also one of the risk factors for penis cancer.

Therefore, it is up to the doctor to evaluate the basic situation, whether infection often occurs, whether it can be easily turned up, etc., and to make a decision according to his own wishes.

2. Phimosis

Most infants have physiological phimosis.

With the growth and development of the baby, the penis will gradually develop, the foreskin mouth will gradually become larger, and the foreskin will retreat upward to expose the penis head, thus gradually separating the foreskin from the penis head.

Therefore, the physiological phimosis of infants does not require surgery.

3. Surgical foreskin

Children’s redundant prepuce and phimosis are mostly physiological phenomena. Pay attention to observation. Generally, surgery is not required, but surgery is required if redundant prepuce or phimosis causes the following conditions.

  1. Repeated urethral infection or dermatitis, etc.;

  2. Difficulty in urination, for example, when urinating, the urine line is very small, and urine can only be urinated drop by drop.

  3. Pubertal phimosis.

4. Buried penis and concealed penis

Buried penis is mainly found in those [small fat lumps], and the most effective measure is to lose weight. When abdominal fat decreases, penis will naturally be exposed.

Concealed penis should be treated by surgery in time because it will hinder the development of penis.

How should baby’s phimosis and foreskin be cared for?

We have just said that most infants and young children may have physiological phimosis and redundant foreskin. Parents are relieved, but at the same time they will also wonder, should such foreskin pay attention to some what at ordinary times? At ordinary times, how can better prevent urethral infection and dermatitis by nursing the baby’s foreskin?

Step 1: Cleaning

When the baby is young, parents are needed to help the baby clean the foreskin. When the baby is older and can take a bath by himself, parents should teach the child how to clean the foreskin and keep the foreskin healthy.

Cleaning methods:

Pull the foreskin back gently, Be careful not to force yourself, Otherwise, it will hurt the baby’s delicate skin, Causing pain, bleeding and even tearing. Then clean the inner side of the foreskin with mild bath lotion and clear water suitable for the baby. No special cleaner or cotton swabs are needed. Pay attention to thoroughly clean the inner side of the foreskin and ensure that it is dried in time. Finally, remember to gently turn the foreskin back and cover the penis head.

2. Pay Attention to [Foreign Bodies]

Daily need to pay attention to the baby’s foreskin whether there is a small [foreign body], if there is, it is necessary to be careful whether there is inflammation. Usually if the baby has no abnormal situation, pay attention to every day when bathing the foreskin scale clean.

STEP 3 Pay Attention to Urination

When the baby urinates, he should pay a little attention to the speed of urine outflow, the amount of urine, and whether the urethral orifice will bulge a bag, etc. to confirm whether there is dysuria caused by too thin urethral orifice.

If the baby’s urination frequency obviously increases, urination is urgent, and urination cries, etc., it is necessary to pay attention to whether it is caused by urinary tract infection. If urethritis or foreskin adhesion occurs repeatedly, you need to go to the hospital.

4. Postoperative care

For babies who have undergone circumcision, they usually need to keep clean to avoid infection within a few days after the operation. After the circumcision is cured, they generally do not need additional care and do a good job of daily cleaning.

保持清洁更重要

Prepuce is too long and phimosis is a common condition in children.

The phimosis of infants is mostly physiological phimosis. With the development of penis, foreskin and penis head can be separated naturally. Attention should be paid to the development without surgery.

Whether foreskin should be cut or not should be treated according to one’s own situation.

For most people, it is more important to keep the pudendum clean.
