Which acne drugs should be used with caution during pregnancy?

In fact, high-frequency words such as [salicylic acid] and [tretinoin] used to treat acne at ordinary times, and drugs or skin care products containing these ingredients should be used carefully during pregnancy.

1. Oral isotretinoin is absolutely forbidden to take

Isotretinoin has definite teratogenic effect, which can lead to developmental malformation of craniofacial region, central nervous system, cardiovascular system, thymus gland and parathyroid gland.

If you are already taking isotretinoin and plan to have a baby, please consider pregnancy at least half a year after stopping the drug.

2. Drugs or skin care products containing tretinoin, depending on the specific type

Tretinoin-containing drugs

Retinoic acid and attapalin are mainly used to regulate keratinogenesis and cell differentiation and fight inflammation. Recent statistical analysis has ruled out the correlation between spontaneous abortion rate, congenital malformation, premature infants and increase in low birth weight. However, it is generally recommended to avoid using these drugs as a precaution.

Tazarotene is another tretinoin drug, which is explicitly prohibited during pregnancy.

Skin care products containing tretinoin

Retinoic acid is not allowed to be added to skin care products, but retinoic acid derivatives such as tretinaldehyde, tretinoin and tretinol will appear in skin care products. Their concentration in skin care products is relatively low and their percutaneous absorption is limited. If they have been used, there is no need to worry about it, but in principle, they should also be avoided during pregnancy as much as possible.

3. Drugs or skin care products containing salicylic acid have low risk of local use in a short period of time.

Experimental studies on rats have found that systematic administration of salicylic acid (rather than external application) during pregnancy can lead to fetal malformation.

Salicylic acid is a common cutin dissolving agent. High concentration of salicylic acid can directly play the role of cutin exfoliation. The concentration in drugs or skin care products will not exceed 2%. If it is used in a short period of time and is limited to local areas, the risk during pregnancy is not high.

Mild to moderate acne

Generally speaking, for mild to moderate acne with acne as the main manifestation, it is recommended to use external drugs locally for acne or acne.

Erythromycin and clindamycin are common antibiotics, which can inhibit inflammation and reduce propionibacterium acne in sebaceous gland by inhibiting protein synthesis of bacteria. Combined application of benzoyl peroxide can reduce the occurrence of bacterial drug resistance and improve therapeutic effect.

Moderate or above acne

For moderate or above acne with inflammatory papules, nodules and even cysts as the main manifestations, it is recommended to combine oral drugs with external drugs for treatment. Oral drugs are used for [fire fighting], while external drugs are used to smoothly control symptoms.

Erythromycin is not easy to pass through the placenta. It is generally believed that it is safe to use during pregnancy and is the first choice antibiotic for the treatment of severe inflammatory acne during pregnancy.

Cefalexin does not cause fetal defects in animal studies, but data in humans are currently insufficient. The disadvantage is that it may cause Staphylococcus aureus resistance. Amoxicillin has an effect on acne with poor curative effect, but may cause nausea and vomiting symptoms.
